If you forget your password, it is possible to reset your credentials from www.MyPlanConnection.com.  


  • You will then be prompted to enter your username.


  • After entering a correct username, you will then be asked one of the security questions you previously established.  If you have never established security questions, you will not be able to retrieve your password.  Instead, please call us at 800.878.5220.  If we are able to validate your identity, we will reset your password and help you access your online account.  


  • After properly answering the security question, you will receive an email at your primary email address of record with a link to reset your password.  The link will remain active for 24 hours.  If your primary email address is no longer available to you, you will need to contact us at 800.878.5220 for help resetting your password.
