Simply click here and fill out all required fields, or go to https://myplanconnection.com and take the following steps to upload your documents:
1. Click on "Plan Participant."
2. Next, click on "Need Help? Find Answers Here."
3. Then, click on "New Ticket."
4. Finally, complete the new ticket with the requested information, attach your documents and click "Submit."
5. You're done! Your ticket will automatically create an action item for our Transactions department and will be sent along for processing.
