ROLL MONEY INTO YOUR PLAN
*Subject to the terms & conditions of your Plan.
- Log into your MyPlanConnection.com account
- Click My Account on the top-right of your screen
- Select Forms
- Expand the drop-down menu labeled Select form
- Select Rollover Form: rollover $ from your old plan or IRA
- Click Get Results
- Once your form has been built, click Open Form and print the form.
- Submit the completed form and a copy of your last quarterly statement from your prior account. *If you are rolling money in from more than one prior plan, you will need to repeat these steps for each of your prior plans.
- For the fastest processing, please send the completed rollover form and a copy of your last quarterly statement from your prior account through our online portal by clicking here.
- If your prior plan requires a Letter of Acceptance, please indicate this on the rollover form. Once your documents have been reviewed and determined to be in good ordered, we can then request a Letter of Acceptance.
- Monitor your emails (including your junk folder) for the next steps to complete your rollover. *Do not request a payment from your prior provider until you have received our email with your Rollover Payment Instructions.
