ROLL MONEY INTO YOUR PLAN

*Subject to the terms & conditions of your Plan.


  1. Log into your MyPlanConnection.com account

  2. Click My Account on the top-right of your screen

  3. Select Forms

  4. Expand the drop-down menu labeled Select form
    • Select Rollover Form: rollover $ from your old plan or IRA
    • Click Get Results

  5. Once your form has been built, click Open Form and print the form.

  6. Submit the completed form and a copy of your last quarterly statement from your prior account. *If you are rolling money in from more than one prior plan, you will need to repeat these steps for each of your prior plans.
    • For the fastest processing, please send the completed rollover form and a copy of your last quarterly statement from your prior account through our online portal by clicking here.
    • If your prior plan requires a Letter of Acceptance, please indicate this on the rollover form. Once your documents have been reviewed and determined to be in good ordered, we can then request a Letter of Acceptance.

  7. Monitor your emails for the next steps to complete your rollover.
    • If you do not receive the next steps within 3 business days of submitting your preliminary documents, please contact Ameritas.
