Purpose
To ensure all Deconversion Authorization Forms are reviewed for completeness, accuracy, and readiness prior to scheduling deconversion timelines and initiating asset transfer.
Scope
This applies to:
- Adopting Employer Transfer forms
- Individual Plan Transfer forms
Step 1: Confirm Receipt & Initial Status
- Acknowledge form has been received.
- Determine status:
- Partially Signed → Adopter only
- Fully Signed → Adopter + Plan Sponsor (if applicable)
- Fully Signed → Individual Plan Form (Authorized Signer)
- Save form to client folder:
- DeconversionAuthorizationForm-[PlanID]_Partially Signed
- DeconversionAuthorizationForm-[PlanID]_Fully Signed
Step 2: Review Deconversion Authorization Form
Plan Information
- Adopting Employer name is completed
- Plan Name reflects:
- MEP or PEP (if applicable)
- Plan ID is accurate (cross-check with PlanConnect)
- Requested Deconversion Date:
- If requested date is not feasible, use next available scheduling window (Refer to Deconversion Calendar.)
- Must be at least 90 days out
- Last Payroll Date is entered
Deconversion Type
Confirm one option is selected and aligns with intent:
- Establishing Single Employer Plan
- Transfer (merge) to another plan
- Change of Service Provider Only
If transferring or merging:
- New Plan Name is listed
- New Plan EIN is provided
- New Plan #
May require Final 5500 depending on:
- Plan merger → Required
- Service provider change → Not required
New Provider Information
Verify:
- New recordkeeper name is completed
- Contact details are included
Plan Asset Types
- Section may be incomplete → acceptable
- Asset details will be validated via:
- Relius Due Diligence Report
Deconversion Details (CRITICAL STEP)
This section determines how assets move.
Transfer Method
Ensure one of the following is selected:
- Full liquidation (wire)
- Transfer in-kind (TIK)
- Combination of both
Investment Instructions
If partial TIK selected:
- Ticker AND investment name must be listed
If full TIK:
- Investment table not required
Reinvestment Elections
- Mapping to like funds OR
- New investment elections selected
Wire Instructions
Verify:
- Bank Name
- ABA Number (9 digits)
- Account Number
- Account Name
- FFC/OBI Bank Name details:
- Typically reflects Plan or Adopter name
- FFC/OBI Account Number
Step 3: Required Paperwork
- Section may not always be completed → verify if documents were attached to form submission email.
- Includes:
- Blackout Notice
- Adoption Agreement
- Self Directed Account Transfer Forms
- Copy of Board Resolution removing current Directed Trustee as Directed Trustee (Individual Plan Transfer)
Step 4: Fees Section
- Completion not required to proceed
- Note:
- Final invoice will be issued
- If the deconversion fees are not paid prior to the start of the Blackout Period, the Plan
reserves the right to assess such fees from available Plan assets, including forfeitures, revenue sharing accounts and participant accounts, as allowed by the Plan document.
Step 5: Signatures & Authorization
Adopting Employer Form
- Must include:
- Adopting Employer signature + date
- Plan Sponsor signature:
- May be obtained later after timeline is sent. Form must be sent via DocuSign to the Plan Sponsor.
Individual Plan Form
- Only one authorized signer required
General Rules
- Electronic signatures are acceptable.
- Must include confirmation page (for custodians)
Step 6: Validate Authorized Signer
- Confirm signer aligns with PlanConnect (PC), if possible
- If mismatch:
- Flag but note:
- Internal process may allow signer if listed in PC
- Custodians may enforce stricter requirements
- Flag but note:
NIGO (Not In Good Order) Handling
If any required fields are missing or unclear:
- Do NOT proceed with scheduling
- Escalate to Jim for guidance and/or client follow-up
- Clearly document:
- What is missing
- What needs correction
Post-Review Actions
Save & Document
- Save finalized form in:
- zzTransfer Out folder
Pull Supporting Reports
From Relius:
- Census Report
- Due Diligence Report
- SDB / Stable Value Fund report (if applicable)
From Crystal:
- Loan Report (if applicable)
Step 7. Readiness for Scheduling
Form is considered “In Good Order” when:
- All required sections are complete
- Deconversion method is clearly defined
- Signatures meet minimum requirements
- No conflicting or missing information
